The admissions process managed by UGC for medical faculties is very competitive and typically requires high (more than 1.9 Z-score) in biological science subjects (Biology, Physics, Chemistry). Hence only the best students can enter state medical faulties in Sri Lanka.
Based on Z-score values of the 2021 A/L Examination (for 2021-2022 university admissions), the following universities have the highest-ranking medical faculties in the country:

This is the oldest medical faculty in Sri Lanka. The teaching hospital associated with the University of Colombo Medical Faculty is the Colombo National Hospital.
2. Faculty of Medicine, University of Peradeniya, Peradeniya (Z-score 2.1807)
Coming in at a close second is the University of Peradeniya, with a z score of 2.1807. This university is located in the central province of Sri Lanka, in the beautiful Peradeniya university complex, and has a reputation for producing some of the best doctors in the country. The teaching hospital associated with the University of Peradeniya Medical Faculty is the Peradeniya Teaching Hospital.
3. Faculty of Medical Sciences, University of Sri Jayewardenepura, Gangodawila, Nugedoda (Z-score 2.1282)
The University of Sri Jayewardenepura rounds out the top three with a z score of 2.1282. Located in the western province at Nugegoda, this university is known for its commitment to providing high-quality medical education to its students. 4. Faculty of Medicine, University of Kelaniya (z score: 2.0511)
The University of Kelaniya Medical Faculty started as a Private Medical College and later converted to a fully-fledged state medical faculty. The Ragama (Colombo North) Teaching Hospital is associated with the University of Kelaniya.
5. Faculty of Medicine, University of Jaffna, Jaffna (z score: 1.9284)
The University of Jaffna got 1.9284 Z-score. The teaching hospital associated with the University of Jaffna is the Jaffna Teaching Hospital.
6. Faculty of Medicine, University of Ruhuna, Karapitiya, Galle. (z score: 1.9632)
The University of Ruhuna has a z-score of 1.9632. The teaching hospital associated with the University of Ruhuna is the Karapitiya Teaching Hospital.
7. Faculty of Medicine, University of Moratuwa, Kalutara (Z score 1.9220)
The District General Hospital in Kalutara is the official teaching hospital for the Faculty of Medicine. Clinical teaching is set to begin in 2023. This medical faculty was established in 2020 and still they are located in the Moratuwa University complex at Katubedda.
8. Faculty of Health Care Sciences, Eastern University, Batticaloa. (Z score 1.9192)
The teaching hospital associated with Eastern University, Sri Lanka is the Batticaloa Teaching Hospital.
9. Faculty of Medicine & Allied Sciences, Rajarata University of in Saliyapura, Anuradhapura (Z score 1.9330)
The teaching hospital associated with Rajarata Medical Faculty is Anuradhapura Teaching Hospital.
10. Faculty of Medicine, Sabaragamuwa University, Ratnapura (Z score 1.9256)
The teaching hospital associated with the Sabaragamuwa University of Sri Lanka is the Ratnapura Teaching Hospital. This medical faculty was established in 2018.
11. Faculty of Medicine, Wayamba University, Kuliyapitiya (Z score 1.9313)
The teaching hospital associated with the Wayamba University of Sri Lanka is the Karapitiya Teaching Hospital. This medical faculty was established in 2018. The 1st batch of Medical students of the Faculty of Medicine in the Wayamba University of Sri Lanka commenced their clinical training at the Teaching Hospital Kuliyapitiya in 2021.
